Ghosting when taking a question - "doubtful"

I created a few questions in a lesson. When I test a user taking the lesson the questions show a ghost toggle "doubtful" just below the bottom of the question content, centered, and a left-aligned ghost check mark "doubtful". I have tested three different question types, including "slide", "file upload" and "multiple choice" - the ghosts show on each. This occurs on your hosted Opigno environment. Can you help?
